Abstract
Heart failure (HF) is a chronic, progressive, unique, and complex disease that affects millions of people. Despite the advances in diagnosis and management; morbidity, mortality, and readmission remain great. A comprehensive multi-disciplinary approach for patients hospitalized with this chronic disease requires a competent and strong focus on education of patients and families. It is the advanced practice nurses that will continue to be at the forefront of managing heart failure. With the integration of a multi- disciplinary team of educators, research-based findings, evidence-based practice, and a proper transition home, the advanced practice nurse will provide the most impact for this debilitating disease. Guided by Margaret Newman’s Health as Expanding Consciousness Theory, this scholarly DNP-FNP project focused on the use of a multidisciplinary approach to heart failure education involving the inpatient and outpatient settings in the Twin Cities Metro area. Through this scholarly project, a heart failure care map, to streamline each patient visits education was implemented and evaluated. With the use of the heart failure care map, readmission rates declined and more in-depth education on heart failure was being completed. Advance practice nurse’s reactions to the utilization of the heart failure care map were favorable, as well as evaluation by the bedside nurses at the place of implementation. The heart failure care map may be applied to daily DNP-FNP practice with heart failure patients in any setting.
