Abstract
This paper describes a project to develop a parish nurse practice model incorporating a nursing theory. This model is entitled The H.A.R.T. Model, Holistic Action in Relationships for Transformation. The nursing theory involved in this project is Jean Watson's Theory on Human Caring and Transpersonal Caring.
